Guide to receiving medication assistance from Ghatre Charity
Due to limited capacity for serving new patients, currently we can only accept patients who are in the three provinces of Tehran, Alborz, and Yazd or patients from other provinces who obtain their medications from these three provinces. We hope very soon to be able to serve other provinces as well.
Original prescription and proforma (proforma date should be within the last 7 days)
Medical documents including tests, imaging, hospital records (for cancer patients, pathology is mandatory)
Original doctor's certificate mentioning the patient's name, disease name, medication name, dosage and duration of use, doctor's stamp and signature, along with the date
Original birth certificate and national ID of the applicant (for foreign applicants, lab card or passport or unique ID certificate is mandatory)
Original birth certificate and national ID of the patient's guardian (for foreign applicants, lab card or passport or unique ID certificate is mandatory)
Property document (rental home: lease, personal home: property deed)
